Youth Work Permits – Issue employment certificates tochildren under the regulations of the State Department of Labor. Forms can bepicked up at Social Services or printed from the Internet. After the employercompletes the form, it must be brought to Social Services. 

The permit must be signed by the minorin front of the social service’s staff issuing permits. The minor must bringwith them their birth certificate or legal picture identification.
