What are Advance Health Care Directives?  Advance Health Care Directives are legal documents in which you give written instructions about your health care if, in the future, you cannot speak for yourself.  You can download and print the forms from the North Carolina Secretary of State office by clicking this link!  Simply fill out the forms with the assistance of your family and contact Melanie Rodgers to make an appointment to have them notarized.
